Cioccolata is one of the main antagonists in Part V: Vento Aureo, paired up with his partner Secco. He's a textbook psychopath with sadistic tendencies—the kind of villain who genuinely enjoys inflicting pain.
Before joining up with Diavolo, Cioccolata worked as a doctor, though "worked" is generous. He was kicked out of his hospital about two years before the events of Part V, officially for a medical accident. In reality, though, those "accidents" were anything but. Cioccolata deliberately misdiagnosed healthy patients as sick, then operated on them while deliberately withholding anesthetics so they'd wake up and suffer through the procedure. He got off on watching them die. The whole accident story was just cover for his actual crimes.
It's the kind of backstory that makes clear Diavolo picked this guy for a reason—Cioccolata's the type who'll do whatever twisted work needs doing without hesitation or remorse.
