Eva Heinemann is best known as the spoiled daughter of Eisler Memorial Hospital's director—and Doctor Tenma's fiancée, at least initially. When Tenma gets demoted for defying her father's orders, she drops him without hesitation. Later, after her father is murdered by Johan, she tries to win Tenma back, especially once he's suddenly promoted to Chief of Surgery. He rejects her, though, and that rejection basically ruins her life.
From there, Eva spirals hard. She becomes a heavy drinker, cycles through three divorces, and uses settlement money to fund an increasingly desperate lifestyle. At one point she's so drunk she burns her own house down. She ends up drifting across Germany, caught up tangentially in the investigation surrounding the Monster without ever realizing Tenma's involvement.
What's darker is her obsession with Tenma's suffering. She doesn't actually believe he killed her father—she knows Johan did—but she's given evidence against him to Inspector Lunge anyway, driven purely by spite. Fans often note how twisted her motivation is: she doesn't want him dead or even arrested for the murder itself. She just wants to see him rot in prison for the crime of rejecting her. Her fixation on him is genuinely unhealthy.
By the end of the series, she becomes a major target for Roberto, making her one of the few characters whose life gets actively destroyed by the Monster's orbit in ways that have almost nothing to do with the Monster himself.
