Umeda Hokuto works as the school doctor, which puts him in a unique position—he's one of the few people who knows Ashiya Mizuki is actually a girl. He treats her kind of like a younger sibling or nephew figure, offering advice and looking out for her.
He's gay and connected to several other characters in the series through family ties: he's the uncle of Minami Nanba, and his siblings are Nanba Io (Minami's mom) and Umeda Rio. He's also harboring feelings for an old friend who appears in another one of Hisaya Nakajo's stories.
