Kenzou Tenma is a skilled neurosurgeon at Eisler Memorial Hospital in Düsseldorf—one of those rare doctors whose talent makes him stand out immediately. He's best known for his role in Monster, where a single moral choice derails his entire life. When forced to choose between operating on a gunshot victim (a young boy) or the city's mayor, Tenma picks the kid. It costs him everything: he gets demoted, becomes the prime suspect when the hospital director and other prominent doctors turn up dead, and spends nine years under a cloud of suspicion before learning the horrible truth.
The boy he saved was Johan Liebert. Years later, Tenma discovers that Johan is actually the one who killed all those doctors—and that he's become something far worse in the interim. Wracked with guilt, Tenma becomes obsessed with finding Johan to stop him, convinced that he's personally responsible for unleashing this "monster" on the world.
What complicates the picture is that Tenma genuinely isn't the cold, ambitious type. Despite everything, he still cares about people. His acts of kindness and basic humanity end up influencing nearly everyone he encounters, which makes him oddly compelling even when his life is falling apart. He's trying to fix a catastrophe he feels he created, but he's not willing to become a monster himself to do it.
