Dr. Nako is the village doctor from Cocoyashi Village and was a major figure in Nami's past, though he doesn't appear much after that. He's best known for being the one who gave Nami her iconic pinwheel tattoo—originally done to cover up the scars she got from Arlong.
When the Straw Hats arrive in the village, Nako helps out during the fight against the Arlong Pirates and patches up Zoro, Luffy, Yosaku, and Johnny afterward. It's actually his complaints about the crew not having a doctor that sparked Luffy's decision to recruit one. He wears a distinctive red cross bandana and sunglasses—though the anime tends to edit out the bandana in the English version.
